To complete your child’s enrollment, please review and complete the attached forms, including the Enrollment Information Form, Statement of Commitment, and Enrollment & Payment Agreement. We will also need a copy of their current immunization records. Once we receive the required paperwork and enrollment deposit, your child’s place for the upcoming school year will be secured.
